您的问题在于这些陈述:

   reg[3:0] A, B;
   ...
   for (B = 0; B < 16; B = B + 1)

由于 B 是 '4' 位宽,它永远不会大于或等于 16。当它达到 15 (4'b1111) 时,下一个增量将溢出并使其变为 '0'。

您需要使 B 宽于 4 位。
