if($_FILES['uploaded_file']['error'] === UPLOAD_ERR_OK){

表示没有错误，它是唯一的“无错误”值，您将其视为错误。应该是 != 代替。并且此检查必须在 move_uploaded_file() 之前

HTTP_X_REQUESTED_WITH

这是否意味着您正在尝试通过 XHR 上传文件？你不能。

1) 在您的表单 HTML 中，确保包含enctype.

<form method="POST" action="uploader.php" enctype="multipart/form-data">
    <input type="FILE" name="uploaded_file"/>
</form>

2) 确保uploads/具有正确的文件权限以允许 r/w
