您最大的问题是您的验证代码中没有正确的表单名称（或正确的字段名称）。

JavaScript 字符串中不能有未转义的换行符。检查您的 JavaScript 控制台，您可能会遇到语法错误。该错误是onsubmit未运行的原因。

另外，如建议的那样，不要使用document.write，只需用 HTML 正常编写表单，并使用 JavaScript仅添加POST url。
